Ladder rungs keep their name but scale to the source's aspect, so for a source wider than 16:9 (960x360: the 360p rung is 640x240) the name derived from a rendition's coded size isn't the rung's, and the paired packet was missed. Each rung is minted under RenditionTrackID(i); look it up by that.
